Each week, we will provide education and tips to incorporate into your lifestyle, all centered around a healthy heart! #### Breast Cancer Awareness Month Breast Cancer Awareness Month Knowledge is power, especially when it comes to breast cancer. Empower yourself with information about your risk factors, symptoms, and the importance of early detection. Regular mammograms can make a significant difference. Join us in raising awareness and supporting those affected by breast cancer. #### Celebrate, Educate, Advocate: Ways to Make Epilepsy Awareness Month Count Celebrate, Educate, Advocate: Ways to Make Epilepsy Awareness Month CountEpilepsy Awareness Month, observed in November, aims to raise awareness and understanding of the neurological disorder that affects over 50 million individuals globally. During November, we have the opportunity to raise awareness, learn, and provide support in easy and significant ways. Simple actions can help spread the word and demonstrate your support for people with epilepsy. Here are several ways to express your support to those affected by epilepsy. Share and Wear Purple! Wearing anything purple is a terrific way to start the conversation, as purple is the official color of epilepsy awareness. Wearing purple, whether it be a scarf or sweater, demonstrates your support for those impacted by epilepsy. Post a picture to social media, include a brief statement explaining Epilepsy Awareness Month, and use hashtags like #EpilepsyAwareness to get the word out! Change your Social Media Profile Picture It’s quick and simple to express your support in this way! You can add personalized frames or photos to your profile picture from a variety of nonprofits, such as the Epilepsy Foundation. Friends and family may get curious about this adjustment, which might lead to discussions about raising awareness of epilepsy. Get Seizure First Aid Certified Knowing what to do in the event that someone close to you has a seizure can be an easy way to demonstrate support. Here’s a brief summary: Remain calm, turn them onto their side, support their head, and don’t put anything in their mouth. Consider enrolling in the Epilepsy Foundation New England’s free seizure first aid training to expand your knowledge. You will get ready to confidently help someone having a seizure with this certification training. Support Epilepsy Organizations Numerous charitable organizations dedicate their lives to raising awareness, funding research, and supporting individuals with epilepsy and their families. Think about attending a local awareness event or making a donation to a charity such as the Epilepsy Foundation. Your support helps finance outreach and research initiatives, and even small donations can have a significant impact. Use Purple Ribbons Creatively All month long, you can show your support by pinning a simple purple ribbon to your jacket or purse. Think about creating ribbons to give to loved ones if you’re feeling creative. To spread awareness, you can even distribute them at your place of employment, school, or neighborhood. Organize a Purple Day at Work or School To increase awareness, propose a “Purple Day” where all employees or students dress in purple. To make it entertaining, pair this with a quick informational session, a group lunch, or even a picnic with a purple theme. It’s a simple method to inspire others to participate and get knowledge about epilepsy. Use Social Media to Spread the Word Every post counts! To promote understanding, share myths, or facts about epilepsy. Many people are unaware that over 50 million individuals worldwide are affected by epilepsy, and that anyone can be impacted, regardless of their age or background. Sharing a single post can raise awareness significantly and educate dozens of people. Get in Touch with Someone Impacted by Epilepsy Reach out, offer support, and let someone know you care if you know someone who is impacted by epilepsy. Particularly during Epilepsy Awareness Month, a small note or considerate act can make a huge difference for someone.Spreading knowledge, increasing awareness, and letting people with epilepsy realize they’re not alone are the goals of supporting Epilepsy Awareness Month. We can all contribute to making a difference, whether it’s with a colorful remark or an act of compassion. So let’s support the epilepsy community, educate ourselves, and wear purple in November! #### Check for ticks! Check for ticks!Tick season is in full swing. Here are some tips to #BeTickAware: Dress the part. When possible, wear clothing that keeps ticks off your skin and makes them easier to spot (long sleeve/pants and light colors) in high tick traffic areas. Wear repellent. Use on-skin repellent (DEET, picardin 20%, oil or lemon eucalyptus) and on-clothing repellent (permethrin). Using both can provide double the protection! Always CHECK for ticks! Look everywhere. They love to hide in areas such as the scalp, behind ears, under arms and between fingers and toes, waist, back, belly button and groin. Keep your family and pets safe this tick season by using preventive measures to avoid bites and Lyme disease. #### Complete a vision exam for your young student Complete a vision exam for your young studentDid you know NH state recommends a complete vision exam for all students within first year of entry or transfer and screening for pre-K, K and 1st grade? Please reach out to our BCCP site coordinator Jeanne Baril, LPN at 603-447-3500 or you can reach out through your patient portal to take the next steps in the application process. You may be able to access FREE breast and cervical cancer screening and diagnostic follow-up services if you meet the following requirements: Residence You live in New Hampshire (or bordering York County Maine). Knowing your risk of hereditary cancer can help prevent cancer or detect it sooner. Did you know? Breast. Ovarian. Endometrial. Prostate. Colon. Pancreatic. Kidney: These are a few of the cancers with a hereditary component. Cancers can be inherited from both the mother’s and father’s side of the family. Fathers can pass along the gene for breast cancer to their daughters. That’s why it is so important to gather as much family history as possible. If any of the above cancers or others have appeared in your family history, you may want to consider genetic testing. Genetic testing can help you determine if you are at increased risk of developing certain types of cancer. If your appt does not coincide with your refill needs the most timely way to request a refill is through your pharmacy or through your patient portal at least 4-5 days before you are due to run. This will allow time for your request to be processed. Requests through the Patient Health Portal Patients of SRMG can request prescription renewals by logging into your (patient portal link) This is the preferred and most timely way of requesting a refill. Once you have successfully logged in: Navigate to the “My Health” on the left side of the screen Select the “medication” tab at the top of your screen Scroll down and find the medication you wish to have refilled Fill out the drop downs with your provider and prescription In the body of the message, be sure to confirm your pharmacy Telephone Requests Follow these easy steps to renew authorized refill requests by telephone Call our Main number 603-447-3500 and either dial 216 or ask for the extension 216 for Refills. Leave your name Date of Birth Complete prescription name Name of pharmacy and the prescribing doctor. Any missing information could cause a delay in your prescription request being refilled promptly. Due to occasionally high volume of refill requests, please allow 72 hours (excluding weekends) ***Please notify us prescription renewals at least 3 to 4 days before your prescription is due to run out. We also ask that you remind your provider of any needed renewals during any office visit. Prescription text notifications To make prescription pickups easier, Saco River Medical Group (SRMG) has enrolled in a service that sends a text notification to your smartphone each time a medication is electronically prescribed for you. You must consent to receive texts and have a valid smartphone number entered. Saco River Medical Group will work with your pharmacist to address health concerns and strive for wellness goals. SRMG can activate notifications for you or you can activate them yourself from your patient portal.  For more information, download this pdf for complete details. Prescription text notifications FAQs: Do I need a particular smartphone or app to receive texts? No. All modern touch screen phones— iPhone 6+ and Android 24+ ―can receive prescription notifications. Notifications appear as texts. No special application is needed.  I’ve received a text message from #64556 and the link in the message is from rxinform.org. Is this the notification from my provider? Yes. Texts from #64556 with rxinform.org links are legitimate messages sent on behalf of your provider. If unsure, contact your provider’s practice to confirm.  Do all notifications include drug discounts and educational information? No. If the drug manufacturer provides drug discount information, videos, and educational content, these are included in prescription notifications but may not appear in all messages. Discounts or coupons included may not be honored by all pharmacies.  Can I opt-out of receiving prescription notifications? Yes. When you receive a prescription notification, reply with ‘stop’ or ‘unsubscribe’ to opt-out.  I opted out of receiving prescription notifications. Can I opt back in? Yes. Select the Text Message option in your patient portal account or contact your clinic to enable notifications. See Eligibility and participation for details.  Important to remember: All prescriptions must be current (within the last 3 months) to be considered for a renewal. Please check with your pharmacy before calling our office to check on the status of your refill. (your patient portal will also show the status of your request) You will not receive a call back from our staff unless there is a question about your refill request. We wanted to share with you the complex process this can for your provider be to help you to better understand. A prior authorization (prior auth) is a requirement by your health insurance plan to obtain approval for specified medical services such as certain Prescription medications Diagnostic imaging, Physical therapy Medical equipment. Without this approval, the medical service will not be covered by your insurance. And sometimes prior auth requests are denied by the insurance company. SRMG handles prior auths for our patients – usually for new prescriptions and imaging. See the example below of a typical prior auth process for a medication (please note that the process varies from patient to patient and can be different for each person/situation). Your provider prescribes a new, brand name medication for you, or it may be a medication you have been on chronically, but you have changed insurance plans and now have different pharmacy benefits.  The prescription is sent to the pharmacy The pharmacy runs your claim through your insurance and the claim is rejected because a prior auth is needed The pharmacy contacts SRMG to let us know (this is sent through fax) SMRG contacts your insurance on your behalf to request the prior auth through multiple ways depending on your prescription plan. Typically this is submitted through your chart to your insurance carrier. (we may need to call you to gather for more information) When SRMG receives approval of your prior auth. If the prior auth is denied, we work with your SRMG provider and you to appeal the denial or to find an alternative medication for you that your prescription plan will cover. This process with your insurance company can sometimes take days to complete.  This can be because your plan is requiring more information to validate the need for the medication. Please be assured that SRMG’s prior authorization team is working hard for you behind the scenes to help you get the medicine or care that you need as quickly as they can. Note:  Most insurance plans also require a prior authorization for high-end diagnostic imaging. Please contact your health plan directly for specific details. PRIOR AUTHORIZATION FAQWhat might cause my insurance company to require a prior auth? Formulary change: when your plan decides that there are alternative medications they will cover- they will require your provider to show proof that you have tried the alternative medications that they do cover. A high-tier medication: your plan may offer to cover a lower tier if one is available Exclusions: when your insurance company no longer covers the medication your provider prescribes Quantity: when your insurance plan will only allow a certain quantity of medication – additional quantity may only be covered if considered medically necessary by your plan What are some reasons a prior auth may be rejected? Plan requires the patient to have a trial and failure of their preferred and covered medications first If a medication is excluded, the plan no longer covers that medication Medication quantity or dose exceeds the max allowed through your health plan How can I assist in the prior auth process? You can call the member services number listed on your insurance card or go to their website to confirm which medications are covered without a prior auth on your health plan If it has been more than 4 days since your medication claim rejected at the pharmacy, please call SRMG or log into your patient portal and request to speak with our prior auth team. Please be sure to have your current pharmacy benefits available (or know who your pharmacy benefits go through) and know the name of the medication that rejected at the pharmacy Please be patient. We work with your insurance company and progress can be limited by their processes and guidelines Once a prior auth has been completed and denied, what is the appeal process? Patient or SRMG provider can request an appeal Appeal process begins – and can take 48 hours to 60 days with your health plan Insurance company sends final decision letter to your provider as well as the patient Note:  Most insurance plans also require a prior authorization for high-end diagnostic imaging. Please contact your health plan directly for specific details. 4 tips for easing Prior Authorization Don’t get surprised — Ask  your doctor “Does this usually require prior authorization?” Keep what works — Did this letter work last time? Then keep it on file for next time! Stay a step ahead — If you know you’ll need it, see if your doctor can file a pre-authorization* to avoid delays. Fix what didn’t work — When prior authorization fails, there is an explanation. Use that information to file an appeal. *filing the necessary paperwork before they even ask for it #### Referrals ReferralsSince Saco River Medical Group is an independent medical practice accepting most major insurance plans, you and your physician decide together where you will receive the best care if you need to see a specialist. We work closing with patients in making choices based on specific medical needs, pre existing relationships, convenience, insurance requirements and other person preferences. Our Referral coordinator will work closely with your insurance company in making sure your referral is approved. All referrals are subject to the restrictions/conditions imposed by the insurance company. Once your referral is complete, it will be forwarded to the specialist.  Depending on the insurance company, it may take up to 10 days for a referral to be fully processed and forwarded to the specialist’s office.  HMO insurance companies must approve our request for a referral, assign a referral authorization number, and forward that information to the specialist.  Our Staff is very conscientious in processing referrals as expeditiously as possible. Your referral will be delayed if: Your request was not generated from a recent visit. In this event your request would have to be forwarded to your provider for approval. If you have an HMO insurance and SRMG is not listed as your provider. #### Resources Patients & VisitorsPay Your Bill The Patient Portal makes it easy to pay your bill with a range of options. Natural immunity to a disease can weaken over time. How quickly or slowly this happens depends on the disease. Vaccines contain many harmful ingredients. Vaccines contain ingredients that allow the product to be safely administered. Any substance can be harmful in significantly high doses, even water Vaccines contain ingredients at a dose that is even lower than the dose we are naturally exposed to in our environment Vaccines can cause Autism in childrenScience has not yet determined the cause of autism. The diagnosis of Autism is made during the same age range that children are receiving their routine immunizations. The 1998 study that raised concerns about a possible link between measles-mumps-rubella (MMR) vaccine and autism was retracted by the journal that published it because it was significantly flawed by bad science. There is no evidence to link vaccines as the cause of autism. Not getting vaccinated is ok because enough people have gotten vaccinatedHerd immunity occurs when a large portion of a community is immunized against a contagious disease, reducing the chance of an outbreak. Infants, pregnant women and immunocompromised people who cannot receive vaccines depend on this type of protection. However, if enough people rely on herd immunity as the method of preventing infection from vaccine-preventable diseases, herd immunity will soon disappear. #### Walk-In Team Our ProvidersWalk-In Team Judith BrewsterPA-C Judith has been with Saco River Medical Group since 2000, working in our Walk-In Clinic and providing urgent care and occupational health to our community and visitors. Research shows that a combination of medication and therapy can help treat substance use disorder successfully and help sustain recovery for people struggling with addiction. SUD is also used to prevent or reduce opioid overdose. It is used primarily for addiction to opioids such heroin and prescription pain medication that contain opiates. The medication used for medication assisted treatment functions to normalize brain chemistry, block euphoria and relieve physiological cravings. It allows the body to function without negative and euphoric effects. Research shows that SUD has reduced the need for inpatient detox, improve patient survival, increase retention in treatment, decrease illicit opiate use and other criminal activity among people with substance use disorders, increase the ability to acquire and maintain employment and improve birth outcomes among women who have substance use disorder and are pregnant. The FDA has approved several medications to treat alcohol and opiate use disorders. These medications relieve the withdrawal symptoms and psychological cravings. Medications used for SUD are evidence-based treatment options and do not just substitute one drug for another. Common medications used to treat alcohol disorder include Acamprosate, disulfiram (Antabuse), and naltrexone. They do not cure alcohol use disorder, but are most effective in people who participate in a SUD program. Buprenoprhine and Naltrexone are used to treat opioid use disorders to opioids including heroin, morphine, and codeine, as well as semi-synthetic opioids like oxycodone and hydrocodone. The medications used for medication assisted treatment are safe to use for months, years or even a lifetime. Substance Abuse and Mental Health Services Administration. (Last Updated: 11/29/2021). This is most often when moderate or high-complexity medical issues can occur. During this period, chances of readmission increase if proper transition and quality of care are not provided. Transitional Care Management (TCM) is a paid, preventive wellness program. It was started by the Center for Medicare and Medicaid Services (CMS) to provide patients with post discharge follow up services. The goal of the program is to eliminate gaps in your healthcare to avoid medical errors in followup, by reviewing your discharge instructions with a TCM nurse within 2 days post inpatient admission and a face-to-face visit with your provider 7-14 days after discharge. What can you expect as the patient? You will receive a call from our TCM coordinator. She/he will reconcile any changes in medications and go over discharge instructions with you. Oftentimes, as a patient you may need guidance on what your next steps are after you have been discharged. She/he will schedule you to see your primary care provider, review any new medications or changes in any of your current medications and help to coordinate any follow up appointments you may need. She/he will help to identify any barriers you may have in follow through with the plan of care upon discharge, for example; transportation to get medications or appointments. This will all help to reduce your potential for readmission and support you through the transition from inpatient to outpatient. The program can be applied upon discharge from one of the following settings: Inpatient acute care hospitalInpatient psychiatric hospital Long-term care hospital Nursing home Inpatient rehabilitation center Hospital outpatient observation/partial hospitalization Patients who receive this TCM service typically see as high as an 86% decrease in the odds of readmission compared with patients who did not receive TCM services. You can reach TCM nurse: Monday, Tuesday, Thursday and Friday 8:00am – 4:30pm 603-447-3500 ext. 218 Your patient portal is also available at anytime that is convenient for you.
