- Team lead for one of our primary care teams
- Excellent communication and interpersonal skills
- Have a strong clinical background with primary care (experience preferred)
- Work closely with the Quality Management Coordinator
- Knowledge in CMS quality measures
- Primary care experience preferred
- Strong communications skills
