Chronic Care Management (CCM)
Your Care, Even Between Visits
At Saco River Medical Group, we know your health needs don’t stop when you leave the office. Our care management services provide additional support between appointments to help you stay on track with your health goals and manage ongoing conditions. Depending on your needs, you may benefit from Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or Collaborative Care Management (CoCM).
Chronic Care Management (CCM)
Chronic Care Management (CCM) provides ongoing support for patients living with one or more chronic health conditionsOur care team works with you to develop and follow a personalized care plan, coordinate care with your healthcare providers, help manage medications, and connect you with resources that can support your overall health. CCM helps ensure you have someone keeping an eye on the bigger picture—even between office visits.
Remote Patient Monitoring (RPM)
allows certain health measurements, such as blood pressure, weight, blood glucose, or oxygen levels, to be monitored from the comfort of your home.Your readings can be reviewed by our care team, helping us identify changes in your health sooner and determine when additional support or follow-up may be needed. RPM can provide you with greater confidence in managing your health while helping your care team stay connected to your progress.
Collaborative Care Management (CoCM)
brings your primary care team and behavioral health support together to provide coordinated care for concerns such as depression, anxiety, and other behavioral health needs. With your permission, members of your care team work together to monitor your progress, support your treatment plan, and help connect you with appropriate resources. This integrated approach recognizes that mental and physical health are closely connected.
These services are designed to provide more support, better coordination, and greater connection to your care team between visits. If you think one of these services may be right for you, talk with your Saco River Medical Group provider or care team to learn more.
